Blank vehicle Crash Reports may be picked up at any police station; they also may be obtained at the following website (requires adobe reader); http://www.mass.gov/rmv/forms/21278.pdf 

If you need to download Adobe Reader, comply with the following:

Click on Download Now.
Click the Yellow Continue Box.

Reminder:

                        Where to send completed Crash Reports:

                                        Mail or deliver one copy to the
                                        Police Department in the city or town
                                        where the crash occurred.

                                        Mail one copy to your Insurance Company.

                                        Mail  one copy to the RMV at the following
                                        address:

You must turn-in your completed Crash Report to the Police Department of the Town where the accident occurred before the police copy will be released to you.
