Blank vehicle Crash Reports may be picked up at any police station; they also may be obtained at the following website:

If you need to download Adobe Reader, comply with the following:
- Go to the following website: https://acrobat.adobe.com/us/en/products/pdf-reader.html.
- Click on Free Download.
- Click the yellow Install Now box.
- Follow Instructions to Download Adobe Reader.
Reminder:
Where to send completed Crash Reports:
Mail or deliver one copy to the Police Department in the city or town where the crash occurred.
Mail one copy to your Insurance Company.
Mail one copy to the RMV at the following address:

You must turn-in your completed Crash Report to the Police Department of the Town where the accident occurred before the police copy will be released to you.
